Office of the Registrar

The Registrar’s Office is the repository of all students’ academic records, documents, information and other related materials pertaining thereto. It serves as the main frontline service of the institution as far as curricular and academic matters are concerned. The office handles student admission, enrolment, student load and/or subject sequence, crediting, graduation, transfer and dismissal of a student.  It prepares the academic calendar and consolidates the class and examination schedules, prepares and submits pertinent reports to Commission on Higher Education (CHED), Professional Regulations Commission (PRC) and the Civil Service Commission (CSC).

  • Transcript of Records
  • Transfer Credentials
  • Certificate of Grades
  • Certificate of Enrollment
  • Certificate of Good Moral

Kindly take note of the policy in claiming of documents:

  • No academic records will be released to a representative without a written authority from the student he/she represents.
  • Please present IDs of both student and the authorized representative.
  • The request will not be processed if the student has incomplete academic credentials (e.g. Form 138, Form 137-A, Transfer Credential, Official Transcript of Records, Good Moral, etc.)
  • Transfer Credential is released only once.
